Licensed Practical Nurse (LPN) - Home HealthLocation: Northeast South CarolinaWe are seeking a dedicated Licensed Practical Nurse (LPN) to provide exceptional care to patients in their homes. In this role, you will collaborate with a multidisciplinary team to ensure the highest quality of patient-centered care.Home Health LPN Responsibilities:Maintain professionalism and strong relationships with patients, visitors, and colleagues.Conduct patient revisits under the Team Clinical Manager's supervision.Collaborate with RNs and other team members to ensure coordinated care.Communicate effectively to promote patient-centered care.Demonstrate strong writing, computer, and organizational skills.Identify and address safety issues for staff and patients.Engage empathetically with a diverse patient population.Ensure accurate data collection and record-keeping.Present a neat and professional appearance.Adhere to attendance policies.Home Health LPN Requirements:Minimum of one year of nursing experience; home care experience preferred.Must possess a valid driver's license and automobile insurance as required by law.
