Visiting Angels is actively seeking an enthusiastic individual who can cultivate and strengthen relationships with various community stakeholders in Florida , supporting the Fort Myers territory and surrounding areas . The ideal candidate will be instrumental in identifying opportunities for collaboration and engagement, focusing on promoting our services and serving as a valuable resource for potential clients, families, and support networks.

The Care Coordinator/Liaison plays a crucial role in driving care consultations and sales initiatives, as well as fostering strategic partnerships within the community for our non-medical home care service organization. We are seeking a dynamic individual, ideally with local connections to spearhead our outreach efforts.

Care Coordinator/Liaison Responsibilities:

  • Conduct care consultations with potential clients to understand their needs and develop personalized care plans in collaboration with their families.
  • Cultivate partnerships with referral sources through educational initiatives, presentations, and events, serving as a knowledgeable resource for clients and their support systems.
  • Drive sales and marketing efforts to expand business, leveraging existing relationships and establishing new connections within the community.
  • Provide support and guidance to current clients through quality assurance visits, ensuring the delivery of exceptional care, and offering case management services as necessary.
  • Engage with caregivers to address their individual needs, reinforcing their importance as valued team members.

Care Coordinator/Liaison Requirements:

  • Demonstrated experience in building and nurturing positive relationships to drive business growth.
  • Previous case management experience is strongly desired.
  • Home Care or Healthcare background or familiarity is highly preferred.
  • Ability and willingness to travel regularly throughout Fort Myers and surrounding areas to meet with clients, families, and referral partners is required.

Note: Our Administration Office is located in Bonita Springs, FL. We also have a Satellite Office located in Naples, FL. This position would cover the Fort Myers area.
